At Validus Consulting, our Community Choice Guide (CCG) services help individuals successfully transition from institutional settings into independent or supported living environments. We also assist those who need additional support to remain stable and safe in their current homes. Our team provides personalized, hands-on guidance designed to help clients build a strong foundation for long-term community living.
Community Choice Guides work closely with clients to identify housing options, connect them with vital community resources, coordinate with healthcare and support providers, and assist with required documentation and applications. As a short-term support service, CCGs play a critical role in helping individuals navigate complex systems and access the services they need to thrive.
Authorized to deliver services outlined within a client's care plan, Validus Consulting's Community Choice Guides address a variety of needs, including behavioral support, tenancy stabilization, hoarding mitigation, and decluttering assistance. We collaborate with case managers, landlords, housing agencies, and community partners to help secure safe, stable, and sustainable housing solutions.
While Community Choice Guides do not provide medical treatment or direct financial assistance, they help clients access the programs, resources, and service networks that can meet those needs. Our goal is to empower individuals with the tools, knowledge, and connections necessary to achieve and maintain long-term independence, stability, and quality of life.
