Community Choice Guide (CCG) Services
At Validus Consulting, our Community Choice Guide (CCG) services help individuals successfully transition from institutional settings into independent or supported living environments. We also assist those who need additional support to remain stable and safe in their current homes. Our team provides personalized, hands-on guidance designed to help clients build a strong foundation for long-term community living.
Community Choice Guides work closely with clients to identify housing options, connect them with vital community resources, coordinate with healthcare and support providers, and assist with required documentation and applications. As a short-term support service, CCGs play a critical role in helping individuals navigate complex systems and access the services they need to thrive.
Authorized to deliver services outlined within a client's care plan, Validus Consulting's Community Choice Guides address a variety of needs, including behavioral support, tenancy stabilization, hoarding mitigation, and decluttering assistance. We collaborate with case managers, landlords, housing agencies, and community partners to help secure safe, stable, and sustainable housing solutions.
While Community Choice Guides do not provide medical treatment or direct financial assistance, they help clients access the programs, resources, and service networks that can meet those needs. Our goal is to empower individuals with the tools, knowledge, and connections necessary to achieve and maintain long-term independence, stability, and quality of life.
Referrals
All Referrals MUST contain necessary documents, to be processed:

- Goal Summary
- Service Summary
- Assessment Detail (If applicable)

Transitional Services
In Washington State's Community Choice Guide (CCG) Program, transitional services help eligible individuals move from institutions, nursing facilities, adult family homes, or other care settings into a community-based living arrangement of their choice. The goal is to support a safe and successful transition while promoting independence.
Transitional services may include:
- Locating and securing accessible, affordable housing.
- Assisting with applications, paperwork, identification documents, and benefit updates.
- Coordinating the physical move and arranging relocation services.
- Setting up the new home, including obtaining essential household furnishings, supplies, and utility services when authorized.
- Connecting individuals with medical, community, transportation, and social support services.
- Providing orientation and training to help individuals access community resources such as public transportation, grocery stores, libraries, and community centers.
- Offering intensive one-on-one support before, during, and after the move to help ensure stability in the community.
Transitional services through the Community Choice Guide Program are designed to remove barriers to community living and help individuals establish and maintain a safe, stable home environment.
Community Stabilization
Community stabilization in Washington's Community Choice Guide (CCG) Program refers to short-term, individualized support that helps people remain safely and successfully in their current community living setting and avoid unnecessary placement in a more restrictive environment, such as a nursing facility or other institutional setting.
Community stabilization services may include:
- Identifying and addressing barriers that threaten a person's ability to remain in the community.
- Coordinating housing, medical, behavioral health, and community resources to maintain stable living arrangements.
- Assisting with applications, benefits, identification documents, and other paperwork needed to maintain community supports.
- Helping individuals connect with transportation, social activities, and other community resources that support independence.
- Assisting with relocation to a different community setting when necessary to preserve housing stability and prevent institutionalization.
- Coordinating with family members, caregivers, service providers, housing agencies, medical professionals, and other supports.
Community stabilization helps individuals who are already living in the community maintain safe, stable housing and access needed supports so they can continue living as independently as possible and avoid institutional care.
