  One of the most rewarding parts of my career has been witnessing the resilience of the clients we serve. Every individual has a unique story, and it's a privilege to walk alongside them as they overcome obstacles, gain independence, and achieve goals they once thought were out of reach. Those successes leave a lasting impact on them—and on me.

As Validus' Senior Supervisor, I am equally proud of our CCG team.   One client expressed appreciation after a visit by stating, "Thank you for coming to see me today, it was the highlight of my day." This feedback reflects the importance of regular contact and supportive engagement, particularly for clients who may experience isolation or limited social supports. It demonstrates how consistent CCG services can improve a client's emotional well-being and sense of connection.

  A client who was preparing for discharge from a Skilled Nursing Facility (SNF) faced a significant barrier to a successful transition back into the community. Prior to hospitalization and SNF placement, the client had been experiencing homelessness. The primary goal was to secure safe, stable housing before discharge to prevent the client from returning to homelessness.

The CCG worked diligently to identify appropriate housing options and successfully located a newly constructed apartment in the area preferred by the client. In addition to securing housing, the CCG coordinated the move and assisted with obtaining transitional household items to help the client establish a safe and comfortable living environment.

The client was overwhelmed with gratitude and excitement upon seeing her new apartment. Having experienced homelessness prior to hospitalization, she expressed disbelief that she had been able to obtain such a beautiful place to live. The client repeatedly stated, "I can't believe it. Thank you, and my mother thanks you and thinks you are a gift from heaven."

  Another significant success involved a client living in extremely unsafe housing conditions. The apartment contained garbage piled to the ceiling throughout the kitchen and dining room areas. Although substantial progress was made removing the garbage, the unit remained uninhabitable due to extensive pet waste from a dog and cat using the apartment as a litter box. The client strongly wished to remain in the apartment, making the situation especially challenging.

Through persistent encouragement, problem-solving, and step-by-step support, the CCG helped the client recognize the need for alternative housing and navigate the process of relocating. Ultimately, the client successfully moved into a new apartment that was safer, cleaner, and more conducive to her health and well-being. The impact of this intervention was recognized not only by the client but also by her family. Two of her adult children contacted the CCG to express their gratitude, stating that they had been unable to accomplish what the CCG had helped their mother achieve.

The client continued to recognize the positive changes brought about through this support. During a Christmas-time phone call, she thanked the CCG for helping her through the transition and stated, "I am happy and pleased to be here and you are an angel." 

This outcome highlights the effectiveness of person-centered support, perseverance through difficult circumstances, and the life-changing impact that comprehensive case management services can have on a client's safety, stability, and quality of life.

At the conclusion of the visit, the client expressed appreciation for the support provided and stated, “I am really glad you came. I always feel better for a while after you have been here.”
