Validus Consulting & Services (VCS) is committed to ensuring the privacy of its program participants and safeguarding the confidentiality of all personal information provided. Personal and private health information obtained by VCS staff will be maintained in strict confidence in compliance with all applicable federal and state laws and consequently will not be released or disclosed to anyone outside of VALIDUS without the participant's consent. Program participants have control over what information is shared and used. VCS does not release information to anyone unless the client authorizes it or the law requires or allows it. Some examples of when the law requires or allows VCS to release information are when there is 1) suspected abuse, 2) a medical emergency or threat of harm, 3) a subpoena issued and served to VCS by a court of law for specific records. Even when allowed by a participant's authorization or law, VCS will only use or disclose the minimum amount of information that is necessary to accomplish the required task. Our highest priority is to ensure that we maintain confidentiality wherever possible. VCS does present an Authorization Form for the release and exchange of information, as needed, to facilitate our ability to obtain the necessary information to best assist our participants, enabling us to work effectively with other related agencies and professionals. Upon signing the Authorization Form, the participant gives permission for VCS and the agencies and individuals listed to exchange the specified confidential information. The participant is not required to sign this Release Authorization form in order to obtain services from VCS; however, we may still share information about you to the extent required and allowed by law. VCS further protects participant confidentiality by ensuring that all VCS staff are aware of and adhere to VCS Confidentiality Policies and Procedures, a copy of which is available to participants upon request. VCS ensures that all records and documentation are stored and purged in a secure manner. Participants wishing to access their personal information maintained by VALIDUS or have any questions regarding the Privacy Practices of VALIDUS may contact our office.

To protect your privacy and ensure we can effectively support your goals, all new clients are asked to complete a Release of Information form during the initial Meet & Greet visit. This authorization allows Validus to communicate and share only the information necessary with relevant individuals, agencies, and service providers involved in your care and support. Completing this form is an important first step in helping us coordinate services and work together toward your success.
